Xeroderma Pigmentosum: a literature review

OBJETIVOS: Essa revisão teve por objetivo agrupar estudos referentes ao xeroderma pigmentoso, apresentando seus meios de diagnóstico e as estratégias terapêuticas atuais. MÉTODOS: Foi realizada uma pesquisa nas bases de PubMed e Biblioteca Virtual de Saúde (BVSALUD) utilizando os descritores: ”XERODERMA PIGMENTOSUM”, “XERODERMA PIGMENTOSO”, ”DNA REPAIR”, “SKIN CANCER”, “PHOTOSENSITIVITY”, “NEOPLASIAS CUTÂNEAS” e “FOTOSENSIBILIDADE” nos últimos 10 anos, sendo feita análise e aplicação dos critérios de inclusão e exclusão. Ao final foram selecionados 8 artigos. DISCUSSÃO: Xeroderma Pigmentoso é uma doença autossômica recessiva, sendo de caráter hereditário e que aumenta a prevalência de neoplasias cutâneas e é subdividido em 7 grupos em cujo erro está na proteína NER (Nucleotide Excision Repair) e 1 grupo com defeito na DNA polimerase. Baseado na intensidade e tipo da luz UV e na mutação apresentada o quadro clínico pode variar, sendo mais brando ou mais agressivo, podendo cometer apenas pele, sistema nervoso ou mais de um sistema, causando, também, tumores internos. O diagnóstico é clínico, mas exames moleculares para avaliar a síntese de DNA em células expostas ao sol e análises complementares podem ser feitas para melhor definição da alteração. Em se tratando de estratégias terapêuticas, atualmente o mais utilizado é a privação de luz solar desde a infância, enquanto outras formas de manejo ainda são estudadas. CONCLUSÕES: Conclui-se que o Xeroderma Pigmentoso é uma doença de difícil controle, diagnóstico e manejo, sendo necessários mais estudos a cerca do tema e acesso universal a métodos diagnósticos precocemente, possibilitando uma profilaxia adequada a fim de evitar complicações futuras pela doença.

OBJECTIVES: This review aimed to group studies related to xeroderma pigmentosum, presenting its means of diagnosis and current therapeutic strategies. METHODS: A search was carried out in the PubMed and Virtual Health Library (BVSALUD) databases using the descriptors: "XERODERMA PIGMENTOSUM", "XERODERMA PIGMENTOSO", "DNA REPAIR", "SKIN CANCER", "PHOTOSENSITIVITY", "CUTANEOUS NEOPLASMS" and “PHOTOSENSIBILITY” in the last 10 years, with an analysis and application of the inclusion and exclusion criteria. In the end, 8 articles were selected. DISCUSSION: Xeroderma Pigmentosum is an autosomal recessive disease, with a hereditary character, which increases the prevalence of skin neoplasms and is subdivided into 7 groups whose error is in the NER protein (Nucleotide Excision Repair) and 1 group with a defect in DNA polymerase. Based on the intensity and type of UV light and on the mutation presented, the clinical picture can vary, being milder or more aggressive, affecting only the skin, nervous system or more than one system, also causing internal tumors. Diagnosis is clinical, but molecular tests to assess DNA synthesis in sun-exposed cells and complementary analyzes can be done to better define the alteration. When it comes to therapeutic strategies, currently the most used is the deprivation of sunlight since childhood, while other forms of management are still being studied. CONCLUSIONS: It is concluded that Xeroderma Pigmentosum is a disease that is difficult to control, diagnose and manage, requiring further studies on the subject and universal access to early diagnostic methods, enabling adequate prophylaxis in order to avoid future complications caused by the disease.
