O uso de antidepressivos tricíclicos para o tratamento da fibromialgia: uma revisão sistemática

**INTRODUÇÃO:** A fibromialgia é uma condição de dor crônica, com duração superior a 3 meses, que se manifesta geralmente entre a terceira e a sexta década de vida, estando presente em 2% a 8% da população, predominantemente em mulheres. Os sintomas geralmente cursam com dor generalizada, de moderada a forte intensidade e que geralmente é acompanhada de fadiga, depressão, comprometimento cognitivo, alterações do sono, e sintomas neuropáticos como alodínia e hiperalgesia. O tratamento com analgésicos convencionais se mostra, em grande parte dos casos, ineficaz, sendo então realizada a analgesia através de agentes antidepressivos tricíclicos, inibidores da recaptação de serotonina e noradrenalina ou anticonvulsivantes, a depender do caso de cada paciente.**OBJETIVOS: **o objetivo do trabalho foi analisar e compreender o uso de antidepressivos tricíclicos para o tratamento da fibromialgia.**MÉTODOS: **Trata-se de uma revisão sistemática da literatura em que foi realizado um levantamento bibliográfico nas bases de dados Scielo e Pubmed entre os anos de Janeiro de 2010 a Junho de 2020. Analisou-se artigos acerca do tratamento da fibromialgia com antidepressivos tricíclicos. Foi empregado descritores em inglês e português, sendo esses: fibromialgia, tratamento fibromialgia, antidepressivo fibromialgia, antidepressivo tricíclico fibromialgia.**RESULTADOS: **Os estudos mostram tratamento farmacológico da fibromialgia é complexo, muitas vezes ineficaz e pouco tolerada por pacientes, especialmente em monoterapia. Ainda não há um tratamento definitivo, contudo, os antidepressivos são frequentemente utilizados, sendo os tricíclicos destacados como uma opção de tratamento em 20 dos 29 trabalhos analisados, dentre os quais 14 se mostravam favoráveis ao uso da classe. Sua introdução como tratamento, especialmente associada a anticonvulsivantes, como a pregabalina, promove melhora clínica significativa dos sintomas nos pacientes. A amitriptilina foi a mais analisada pelos pesquisadores.**CONCLUSÕES:** A fibromialgia é uma doença crônica caracterizada principalmente por dor generalizada, levando a diversos prejuízos para os indivíduos acometidos. Seu tratamento efetivo é fundamental, pois possibilita melhor qualidade de vida a curto e longo prazo. O tratamento com antidepressivos tricíclicos se mostrou uma opção válida nessa patologia, especialmente quando combinado com outras medicações e, portanto, deve ser considerado na prática clínica.
