The aim of this study was to analyze the threats and opportunities facing the Health Economic-Industrial Complex (HEIC) based on Brazil's experience with the COVID-19 pandemic and the expansion and accumulation strategies of large transnational companies and national states, from the perspective of industry 4.0 in the field of health. To this end, a narrative review was carried out in digital academic databases that encompassed three thematic blocks: HEIC; science, technology, innovation and production in health; and the international political economy of health. Based on the literature studied, capacities and vulnerabilities demonstrated by HEIC during the COVID-19 pandemic and threats and opportunities for Scientific, Technological and Innovation Institutions (STIs) were identified.
