FUNCTIONAL SPIROMETRIC EVALUATION IN PATIENTS WITH JUVENILE IDIOPATHIC ARTHRITIS IN A REFERRAL OUTPATIENT CLINIC IN THE CITY OF SALVADOR-BAHIA - PRELIMINARY RESULTS

Juvenile Idiopathic Arthritis (JIA) is a rheumatologic disease defined as chronic arthritis that began before the age of 16 years. JIA can be a serious and disabling clinical condition, from the destruction or impairment of joint function. In the literature, there are several studies evaluating pulmonary involvement in adults with rheumatoid arthritis (RA), however, there are few references in the literature on pulmonary involvement in pediatric patients with JIA. The aim of the present study was to evaluate the pulmonary function of pediatric patients with JIA.

Materials and methods

Cross-sectional, descriptive study; 25 children and adolescents with JIA were evaluated in an outpatient reference clinic in Pediatric Rheumatology at the HUPES complex of the Federal University of Bahia, in Salvador, Bahia, between March 2017 and June 2019. A questionnaire containing sociodemographic data and a Spirometric test was applied, according to the recommendations of the American Thoracic Society (ATS) and according to the Guidelines for Pulmonary Function Tests of the Brazilian Society of Pulmonology and Tisiology of 2002. The variables analyzed were: Forced Vital Capacity (FVC), Forced Expiratory Volume in the first second (FEV1) , FEV1/FVC and Forced Expiratory Flow between 25 and 75% of FVC (FEF25-75%). The central tendency measures were evaluated and the Mann Whitney test and the Spearman test were used for correlation analysis.

Results

The mean age (± SD) of the patients was 11.6 (± 3.4) years and the mean (± SD) of the BMI was 18.2 (± 3.1); 64% of the patients were female. When we compared the medians of the spirometric values namely, predicted FVC, predicted FEV1, FV1 / FVC predicted, FEF25-75% predicted, according to disease activity or not, we observed the following values, respectively: 104 x 89; p = 0.16; 109 x 95.5; p = 0.08; 106 x 104.5; p = 0.051; 131 x 123; p = 0.34. Significant and negative association was observed between spirometric values as% of predicted FVC, FEV1, FEV1 / FVC, FEF25-75% and current age, respectively: -0.5 and p = 0.02; -0.4 and p = 0.03; -0.5 and value of p = 0.02; -0.4 and p = 0.03.

Conclusion

We didn't observe differences between spirometric values according to the activity of the disease , but we found association of the spirometric values with the current age of the patient.
